PRESSURE SYSTEM FAILURE LEADS TO LARGEST EVER CORPORATE MANSLAUGHTER FINE

An investigation found an explosion resulted from a screw connection to an autoclave locking ring which secured a door to a machine. The autoclave had been opened numerous times whilst under pressure and sometimes forced. The Company concerned knew of this but did not fix the fault fully.

A fine of £500,000 as the Company was found guilty of Corporate Manslaughter.

The Company is now in Administration!

The above raises the following questions:

  • Do you undertake regular maintenance and act on any faults found post inspection?
